Iatrogenic hypoparathyroidism and hypocalcemia are frequent complications in patients undergone head and neck surgery. Symptoms of hypocalcemia; such as bronchospasm should be differentiated from bronchial obstructive symptoms of chronic obstructive pulmonary disease (COPD). We present a case with COPD and hypoparathyroidism resulted in hypocalcemic bronchospasm. Symptoms of the patient was resisted during bronchodilator therapy and only resolved by emergency treatment of hypocalcemia.
